Nick Isiekwe will start at No 5 lock for England against the Springboks at Ellis Park on Saturday.
The 20-year-old, who has been in superb form for Saracens, earns his first Test start after Joe Launchbury was ruled out through injury. He will partner Maro Itoje in the second row.
Tom Curry, who played with Isiekwe for England U20 last year, starts at openside flank, with Chris Robshaw at blindside and Billy Vunipola at No 8.
Hooker Jamie George will scrum down alongside Mako Vunipola and Kyle Sinckler in the front row.
Coach Eddie Jones has tweaked the backline, with Elliot Daly shifting to fullback and Mike Brown to the left wing. Jonny May is on the right wing, while captain Owen Farrell and Henry Slade team up in midfield and George Ford and Ben Youngs resume their halfback partnership.
There is no room in the match 23 for Danny Cipriani.
Loose forward Brad Shields and scrumhalf Ben Spencer will make their Test debuts if they come off the bench.
