SA-born hooker Rob Herring comes into the Ireland matchday 23, skippered by Caelan Doris, to face the Springboks in the series decider in Durban on Saturday.
Head coach Andy Farrell on Thursday named the lineup to face the Boks in the second and final Test at Kings Park.
Herring features on the bench in place of Ronan Kelleher, who has been promoted to the starting XV for the injured Dan Sheehan.
Doris captains the team for the second time as the Irish bid to level the series after going down 27-20 to the world champions in Pretoria last week.

After making his Test debut last week, Jamie Osborne continues at fullback in an unchanged back three alongside James Lowe and Calvin Nash.
In midfield, Garry Ringrose comes into the side to partner Robbie Henshaw, with Bundee Aki ruled out with a shoulder injury, while there is an all-Munster halfback pairing in scrumhalf Conor Murray and Jack Crowley.
Up front, Andrew Porter and Tadhg Furlong will pack down either side of Kelleher, with Joe McCarthy and James Ryan named in the second row. Tadhg Beirne switches to blindside flanker, with Josh van der Flier at openside and Doris captaining from the back of the scrum.
There are strong options for Farrell to call upon from the bench, with Herring joined by Cian Healy, Finlay Bealham, Ryan Baird and Peter O’Mahony providing forward cover, as Caolin Blade, Ciaran Frawley and Stuart McCloskey complete the reinforcements.
