Fred Zeilinga will start at fullback when the Cheetahs face Connacht on Saturday.
Zeilinga takes over from Sergeal Petersen at 15, who has joined the Stormers, while Nico Lee and Tertius Kruger will form a new midfield pairing.
One other change to the backline sees Luther Obi come in on the wing for Raymond Rhule, who is now with the Boks.
Among the forwards, Junior Pokomela will start at No 8 in the absence of Springbok Oupa Mohoje, while Jasper Wiese will make his Pro14 debut at flank.
Meanwhile, Johan Coetzee will start at tighthead prop, with Carl Wegner coming into the second row.
